How to Apply

 Get an Application 

 Ask for a paper copy at the ADFAC office (1051 Oak Ridge Turnpike, Oak Ridge, TN | Map It); or

 Ask for a paper copy at any of our community events (Calendar); or

 Download an electronic PDF copy (Application).

 Gather Documents

1. Completed Page 1 and Page 2 of Application
2. Proof of current Total Monthly Household Income from occupants (i.e. Previous year’s Federal Income Tax return, pay stubs from the last 30 days and/or an annual benefits statement for the current year)
3. Proof of payment of previous year’s City and County Property Tax
4. Proof of Homeownership/Warranty Deed
5. Photo Identification of residents
6. Most recent Electric Bill
7. Last three months Bank Statements

Rented homes must also submit:

Copies of the required documents must be submitted with a completed application at the time of submission for your home to be considered. Submission of a completed application and documentation does not guarantee participation.
